This standard is specially formulated in accordance with the "Law of the People's Republic of China on the Prevention and Control of Occupational Diseases". In case of any inconsistency between the original standard GB/T18200-2000 and this standard, this standard shall prevail.
This standard is used in conjunction with the reporting system for radiation diseases stipulated in the "Regulations on Health Management of Radiation Workers". This standard is proposed and managed by the Ministry of Health of the People's Republic of China. The main drafting unit of this standard: Institute of Radiation and Nuclear Safety Medicine, Chinese Center for Disease Control and Prevention. The drafters of this standard are Tan Shaozhi, Chang Shiqin, Wang Youfan and Wang Jingying. The Ministry of Health of the People's Republic of China is responsible for interpreting this standard. 1 Scope

2 Requirements for filling in the "Occupational Radiation Disease Report Form", 2.2
2.2.1 About diagnosis
If a person suffers from two or more radiation diseases at the same time, only one report form shall be filled out. 2.2.2 Regarding exposure history
, the cause of exposure and a brief course of events should be stated. If it is external exposure, the type of radiation should be stated, whether it is whole-body or local, single or multiple exposure, or chronic low-dose exposure. If it is internal exposure, the name of the radionuclide, the route of entry into the body, and the amount should be stated. 2.2.3 Regarding exposure dose
, the absorbed dose of the whole body and/or organ (tissue) (specify the measured value or estimated value) and/or the estimated result of biological dose should be stated. 2.2.4 Regarding treatment measures
, only fill in the main treatment measures that play a key role. 5 Regarding current health status and treatment
, write the prognosis estimate (good, better or bad) or outcome according to the patient's condition when filling in the report form, such as cure, improvement, disability (depending on the assessment results, death, and continuing to engage in radiation work, temporary or permanent transfer from radiation work position, etc.
